What uric acid is
Uric acid is a waste product. Your body makes it when it breaks down purines, compounds found in your own cells and in foods like organ meats, anchovies, sardines, and beer.
Normally the kidneys clear it and levels stay in range. Two things can push it up: producing more than usual, or clearing less than usual. Diet contributes, but for most people the bigger driver is how efficiently the kidneys excrete it, which is why two people eating the same food can end up at very different levels.
When uric acid climbs above the point where it stays dissolved in blood, it can form sharp crystals. When those crystals settle in a joint, that is gout.
The gap between "elevated" and "gout"
There is a stage before gout that most people never hear about: hyperuricemia, meaning uric acid above the normal range without any attack having happened yet.
Plenty of people sit there for years. Some never develop gout at all. Others do. It usually shows up on routine bloodwork, gets noted, and gets forgotten.
That in-between group is exactly who the quercetin research looked at, and it is why this study gets misquoted so often.
What the 2016 trial did
Shi and Williamson, working at the University of Leeds, published their results in the British Journal of Nutrition.
The design. Randomized, double-blinded, placebo-controlled, crossover. Crossover means every participant took both quercetin and placebo in different phases, so each person served as their own comparison. That removes a lot of the noise you get when comparing separate groups.
Who. 22 healthy men, ages 19 to 60, with plasma uric acid in the elevated but not gouty range.
The dose. 500 mg of quercetin per day.
The length. Four weeks on treatment, then a four-week washout before switching.
The result. Plasma uric acid dropped by 26.5 µmol/L compared with placebo, roughly an 8% reduction. The result was statistically significant (P=0.008).
What the trial did not show
This is the part that gets dropped when the study gets repeated online.
It was not a gout study. No participant had gout. The trial measured a blood marker in people with elevated levels, not attacks, not pain, not frequency of flares.
It did nothing for normal levels. Participants whose uric acid was already in range saw no reduction. Quercetin did not push healthy numbers lower, which is reassuring and also means it is not doing anything useful if your levels are fine.
It was small. Twenty-two participants, all male, four weeks. That is enough to be a real signal and not enough to be a guideline.
It is not a substitute for treatment. Gout has established medical management, including drugs that lower uric acid and drugs that handle acute attacks. Nothing in this trial suggests replacing any of that. If you have been diagnosed, that conversation belongs with your doctor.
An 8% reduction over four weeks in a small crossover trial is a modest, well-measured finding. Treating it as more than that is where the internet usually goes wrong.
Why 500 mg keeps coming up
The dose is not arbitrary. 500 mg per day is what the trial used, and it is the number that later discussions of quercetin and uric acid keep referring back to.
For context on how that compares to food: quercetin is found in red onions, capers, apples with the skin on, kale, and buckwheat. Red onions are among the densest sources, and typical daily intake from a normal diet lands in the range of tens of milligrams, not hundreds. Reaching 500 mg through food alone is not realistic for most people.
Absorption is the other variable. Plain quercetin is poorly absorbed on its own, which is why some formulations pair it with black pepper extract or use forms designed to improve uptake. A dose on a label and a dose in your bloodstream are not the same number.
What else quercetin is studied for
Uric acid is one thread. Quercetin is a flavonoid, and most of the research on it looks at antioxidant and inflammation-related pathways more broadly.
That is worth knowing because it changes how you think about it. Quercetin is rarely taken as a single-purpose ingredient the way a uric-acid drug would be. It usually shows up as part of a broader daily formula alongside other antioxidants, which is also how it appears in most of the products on the market.
How to read a label
